On-Call Triage Requests 

PLEASE STOP AND READ THIS IMPORTANT INFORMATION BEFORE CONTINUING 

The on-call service is strictly for problems that you are genuinely worried may not be safe to wait for a routine appointment. By continuing with this process you are confirming that you believe it may be unsafe to wait for a routine appointment. 
 
The On-Call Team is made up of GP(s), Physician Associate(s), Nurse Practitioner(s) and supervised Medical Students. You will receive a call back from the team before the surgery closes at 18:30 today. 
 
THE PRACTICE NO LONGER OFFERS ONLINE CONSULTATIONS FOR ROUTINE PROBLEMS 
 
FIRST: Check your symptoms on NHS.UK, with a chemist, or 111 to see if it is recommended you need to urgently consult with your GP Practice. 
 
DO NOT: proceed if it becomes clear that your problem is safe to wait for a routine appointment. 
 
CALL 999: If you are short of breath (to the point you cannot complete sentences), experiencing new severe chest pain, are worried you may be having a stroke, or are actively bleeding. 
 
REMEMBER: most minor illnesses (e.g. sore throats, insect bites, colds, back pain) can be managed by visiting NHS.uk and/or consulting a chemist. 
 
PLEASE NOTE: the on-call team will triage your request. This means they will work with you to either resolve your problem today (if this is necessary to make the situation safe) or signpost you to a routine appointment if the outcome is that the problem can safely wait. 
 
The On-Call team only accept triage requests between 0800 - 1030 in the morning and 1430 – 1630 in the afternoon.  
Outside of these times the online booking system and patient services will not be able to accept further requests.  
If you feel you need urgent help before the next booking window opens please either call 111, visit your local Urgent Care Centre, or attend A&E depending on the urgency and severity of the problem. 
 
PLEASE: Supply a working phone number and be ready to answer the phone at anytime between now and when the practice closes today at 18:30. We cannot accomodate requests for specific call-back times and will not make further attempts to contact you after two failed attempts.
